Mercedes Kilmer revealed to The New York Times and The Associated Press that her father died from pneumonia. This particular lung infection can range from mild to severe. Pneumonia can cause inflammation in the air sacs of the lungs, leading to difficulty breathing, fever and fatigue.
Given Kilmer’s medical history, his battle with pneumonia may have been exacerbated by the long-term effects of his throat cancer and treatments. Those who undergo cancer treatments, especially radiation and chemotherapy, may have weakened lung function or be more susceptible to respiratory infections.

In 2014, Kilmer was diagnosed with throat cancer, though he initially kept his condition private. His struggle became public in 2016 when actor Michael Douglas, who had faced a similar cancer diagnosis, mentioned in an interview that Kilmer was also battling the disease. At first, Kilmer denied the claims, but later admitted on Reddit that he had undergone treatment for throat cancer.
“I did have a healing of cancer, but my tongue is still swollen, although healing all the time,” Kilmer wrote in response to a fan’s question. He later detailed his battle in the Amazon Original documentary, Val, which was released in 2021.
Kilmer’s treatment included chemotherapy, radiation and a tracheotomy, a surgical procedure that creates an opening in the neck to assist with breathing. The procedure left him unable to speak naturally, requiring him to use a voice box. Despite this, Kilmer continued to work in Hollywood, even reprising his role as Iceman in Top Gun: Maverick in 2022, where his dialogue was almost all his voice with the help of artificial intelligence technology. In 2021, Kilmer revealed to People that he was cancer-free.
How does throat cancer affect breathing?
Throat cancer can impact the vocal cords, voice box and surrounding tissues. This makes breathing and swallowing difficult. The effects of a tracheotomy can also increase vulnerability to respiratory illnesses like pneumonia. According to the American Thoracic Society, both tracheostomies and endotracheal tubes increase the chances of pneumonia in patients.
Additionally, individuals with chronic illnesses—including cancer, respiratory conditions or a history of medical treatments that weaken the immune system—face a higher risk of severe complications. However, Kilmer’s family members have not specified a correlation in his death.

A lifelong Christian Scientist, he often spoke about the role of spirituality in his healing. “I prayed, and that was my primary treatment,” he said in a 2017 interview with The Hollywood Reporter. However, he ultimately opted for medical intervention, which helped him achieve remission.
Despite losing his natural speaking ability, Kilmer continued acting and writing, publishing his memoir, I’m Your Huckleberry, in 2020. He reflected on his changed voice, saying, “When I speak now, I sound like Marlon Brando after a couple of bottles of tequila.”
