Tirzepatide vs. Semaglutide Side Effects — How Do They Compare?
Although both semaglutide and tirzepatide medications have a similar effect on the body, the side effects may look a little different.
When selecting a GLP-1 medication for weight loss, you have two options: Semaglutide, which includes popular name brands like Ozempic and Wegovy; and called tirzepatide, which includes name brands Zepbound and Mounjaro. Both varieties are effective for weight loss, but there are some slight differences. One of the more tangible differences between semaglutide and tirzepatide medications are the side effects.
Dr. Randall Turner, dual board-certified physician, D.O. says it all starts with the two medications’ chemical differences. The side effects of tirzepatide and semaglutide medications look different due to their makeup. However, they still provide comparable weight loss results — and are similarly administered as injectables.
“Semaglutide is a GLP-1 receptor agonist. This means it mimics the effects of the GLP-1 hormone to help regulate blood sugar levels and reduce appetite,” he says. “Tirzepatide, on the other hand, is a dual agonist. It works on the GLP-1 and GIP receptors (glucose-dependent insulinotropic polypeptide). Tirzepatide can enhance glucose control and appetite suppression by targeting two different hormonal pathways, giving it a broader action compared to semaglutide.”

The effects of semaglutide medications are can be annoying, but they usually improve.
Through the media boom surrounding semaglutide medications, you’ve likely heard about the side effects. Aside from more serious and long-term symptoms like Ozempic bowel injury and Ozempic face, most side effects are gastrointestinal: Nausea, vomiting, diarrhea, and sometimes constipation. Some also experience fatigue and headaches. Dr. Bruno tells us they start upon upping your dose, but are generally manageable with basic self-care.
“These symptoms usually start when you first increase the dose and tend to improve as your body adjusts,” she says. “So we’re talking a few weeks to months in most cases. Some patients find them mild, while other patients get more intense side effects, especially if they don’t eat properly.”
But the severity also depends on your dosage chart. Dr. Turner reiterates that as long as you’re increasing your dosage incrementally, they don’t tend to be too bad: “While uncomfortable, they often improve as your body adjusts to the medication,” he adds. “For most people, side effects are manageable with gradual dose adjustments.”
Tirzepatide side effects are overall more mild — but stomach issues may be worse
When taking a tirzepatide medication, patients can often expect similar side effects to that of semaglutide. Fatigue, headaches, vomiting, diarrhea and constipation are all in the cards: “Some patients claim the tirzepatide side effects are much more mild than the semaglutide side effects,” Dr. Bruno says.”Like semaglutide, these symptoms kick in early as your body adjusts (and worsen when eating unhealthy foods).”
Stomach issues in particular may be worse
Tirzepatide targets the GLP-1 and GIP receptors, while semaglutide solely works on the GLP-1 receptor. Because of this, Dr. Turner says, some may experience more severe stomach issues. This, however, greatly varies from person to person.
“Due to its dual-action mechanism, some people may experience more pronounced gastrointestinal symptoms compared to semaglutide,” he says. “Rare side effects include pancreatitis and gallbladder issues, and there’s also a theoretical risk of thyroid tumors, similar to semaglutide.”
Tirzepatide may be more effective than semaglutide
Dr. Turner tells us that tirzepatide’s dual receptors make it more effective for weight loss. Studies have also shown this may result in lower blood sugar levels: “On average, tripeptide helps people lose 15–22 percent of their body weight, depending on the dose, compared to 12–15 percent with semaglutide,” Dr. Turner adds. “Both medications are highly effective, but tirzepatide’s dual action may provide an added boost for those looking to manage both diabetes and weight.”
However, results are totally contingent on the individual’s holistic efforts (diet and exercise), Dr. Bruno adds. And everyone’s bodies react differently to different medications. “I always tell my patients that effectiveness also depends on how their body reacts,” she explains. “So it’s not a one-size-fits-all solution.”
Doctor's opinions vary regarding which medication is better
Different doctors may recommend different types of medications. Dr. Bruno, for example, leans towards tirzepatide: “I’d recommend that they start with tirzepatide, as the side effects are milder,” she says. “However, this depends on the individual patient’s body and health history.”
On the other hand, though, Dr. Turner suggests semaglutide as a better starting point. He feels the side effects are overall lesser — but so are the results: “Semaglutide might be a better starting point for someone concerned about side effects because its single-receptor action may result in fewer or less severe gastrointestinal symptoms,” he says.
“However, if weight loss or blood sugar control is the top priority, and the patient is open to managing potentially more substantial side effects, tirzepatide could be the better option.” He adds that working alongside a knowledgeable doctor, such as your primary care physician, will be useful in deciding which medication is right for you.
“Both semaglutide and tirzepatide are highly effective for managing type 2 diabetes and promoting weight loss, but they have unique strengths and side effect profiles,” he says. “The best choice depends on your specific health needs and priorities. Working closely with a healthcare provider can help ensure you pick the option that aligns with your goals while minimizing discomfort from side effects.
