How To Prevent—and Tighten—Loose Skin After Weight Loss, According to a Top Derm

After a lot of grit and determination, you’ve accomplished something you should be incredibly proud of: You lost (or are in the process of losing) a significant amount of weight. And while you feel great about that, there’s one thing you feel not-so great about—loose skin after weight loss.

Extra skin is a common issue, especially for people who drop 100 pounds or more. That’s why we asked a top dermatologist to share everything you need to know about preventing loose skin while you’re losing weight and how to tighten skin after you’ve reached the finish line on your journey.

What causes loose skin after weight loss?

Loose skin following weight loss occurs when the skin’s elastic fibers and supportive structures can’t fully adapt to a reduction in underlying body mass, reveals board-certified and fellowship-trained cosmetic dermatologist Pooja H. Rambhia, MD, FAAD.

What unwanted weight-loss side effects have you experienced?

Though rapid weight loss can exacerbate sagging skin by limiting its adjustment time, she says the degree of laxity is influenced more significantly by four key factors:

  • Total amount of weight lost
  • Aging and its associated decline in collagen and elastin (proteins that support the skin)
  • Genetic determinants of skin elasticity
  • Cumulative environmental skin damage, such as ultraviolet radiation and smoking

How to prevent loose skin after weight loss

To help minimize skin laxity while you’re shedding pounds, aim for gradual and sustainable weight loss, which allows the skin more time to adapt to changes in body composition, advises Dr. Rambhia. Indeed, the rule of thumb is to drop no more than one to two pounds a week.

Incorporating resistance or strength training can also help preserve and build lean muscle mass, providing structural support beneath the skin, she continues. “Staying hydrated, consuming a nutrient-rich diet with sufficient protein [from foods including lean meat, fish, eggs and legumes] and protecting the skin from excessive sun exposure can further support collagen and elastin production, promoting better skin elasticity throughout the weight-loss process.”

How to tighten loose skin after weight loss

Now that you’ve achieved your weight-loss goal, first thing’s first: Congratulations! It wasn’t easy, but you stuck to your plan and you got here.

As for loose skin left in the wake of your weight-loss journey, Dr. Rambhia has a few suggestions: “Aim to eat a diet rich in high-quality protein and nutrients that promote collagen synthesis—like vitamin C, zinc and copper—to help maintain skin structure.” (Discover a few collagen-boosting foods here).

And just as you did while losing weight, keep up with your strength training. It can enhance underlying muscle tone for better contour and support of the skin after significant weight loss, she adds.

Non–surgical treatment options

If natural interventions like tweaking your diet and lifting weights don’t work, there’s more you can try.

Skin tightening

In-office skin-tightening treatments performed by a dermatologist “can deliver more significant results through technologies like radiofrequency (Thermage) and ultrasound (Ultherapy or Sofwave), which tighten skin by delivering controlled thermal energy to precise depths in the skin,” explains Dr. Rambhia.

“These devices can target the deeper superficial muscular aponeurotic system (SMAS) layer, stimulating collagen production and remodeling the skin beyond what [other methods] can achieve,” she says. “Results typically appear over three to six months as new collagen forms, with improvements lasting one to two years with proper maintenance.”

Injections or fillers

Such energy-based treatments can be combined with biostimulatory fillers—injections that boost the body’s collagen-production—like Radiesse or Sculptra.

“Importantly, these procedures do not need to be delayed until weight loss is complete—initiating treatment during the weight-loss process may help maintain skin integrity and reduce the risk of sagging,” Dr. Rambhia adds. One important caveat: “While these approaches are effective for mild to moderate skin laxity, more advanced [cases] may require surgical intervention for optimal results.”

When surgery may be right for you

Surgical intervention is generally considered when skin laxity is severe, with significant excess skin that does not improve with non-invasive treatments, notes Dr. Rambhia. Common surgical procedures include abdominoplasty (tummy tuck), brachioplasty (arm lift), thigh lift and lower body lift—each medical procedure is designed to remove excess skin and restore smoother contours.

Ideal candidates are at or near their goal weight, in good overall health and have maintained a stable weight for several months. Pre-surgical steps to optimize results include achieving a healthy body mass index (BMI), maintaining good nutrition (especially adequate protein intake), avoiding smoking and protecting the skin from sun damage to promote healing and reduce the risk of complications.

“A consultation with a board-certified plastic surgeon is essential to determine the most appropriate surgical plan and ensure safe, tailored care,” Dr. Rambhia encourages.
